NJ Health serves patients with unique medical needs by offering a variety of specialty programs for managing chronic diseases such as Cardiac disease, Pulmonary disease, Cancer, Alzheimer’s disease and Dementia. The programs are designed to meet the specific needs of each patient. Our total team approach and circle of support provides services to both the patient and the family.
Our disease specific experts train our team members to create a customized plan of care for each patient based on their goals and priorities. Our team is trained to provide ongoing education to the patient and their caregivers on their disease that expands their understanding and what to expect as their disease progresses.
Our expertise in pain and symptom management promotes comfort care and quality of life. Our programs are designed to help reduce unnecessary emergency room visits and hospital readmissions. Our goal is to keep patients in the safety and security of their home wherever they call home.
We Honor Veterans Program

NJ Health Hospice and Palliative Care is proud to be a member of the We Honor Veterans Program, a program launched by the National Hospice and Palliative Care Organization in collaboration with the Veterans Administration, (VA). It is designed to meet the unique needs of terminally ill veterans and help improve the care veterans receive from hospice and palliative care providers.
