Part-Time Hospice Social Worker Cumberland County, NJ

Are you a compassionate and dedicated hospice social worker looking to make a meaningful difference in the lives of patients nearing end-of-life and their families?

NJHealth Hospice and Palliative Care is seeking a Part-Time Hospice Social Worker to provide emotional support, counseling, advocacy, and resource coordination for hospice patients and their loved ones throughout Cumberland County.

In this rewarding role, you will:

• Assess psychosocial needs of patients and families

• Provide counseling and emotional support

• Assist with community resources and care planning

• Collaborate with an interdisciplinary hospice team

• Help patients and families navigate end-of-life challenges with dignity and compassion

Qualifications:

• Master's Degree in Social Work (MSW) preferred

• New Jersey Social Work license required

• Hospice or healthcare experience preferred

• Excellent communication and interpersonal skills

• Reliable transportation
